During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. What To Expect
When you arrive for your first appointment, you can expect to be greeted by our friendly reception staff. You will be asked to fill out general treatment consent forms as well as health history forms. You are welcome to print these forms from the website, fill them out, and bring them in to save time prior to your appointment. We ask that you bring your insurance card if you have insurance and a list of your medications.  Please let us know if your doctor requires you to take an antibiotic premedication prior to dental treatment.  We ask that children under the age of 18 be accompanied by a parent or legal guardian for the first appointment.
